Therefore, complete abstinence from ethanol is the most useful measure to control the natural course of ACM [51,56,135]. In fact, patients with ACM who abstain from alcohol have a better long-term prognosis than subjects with idiopathic dilated CMP [54]. Out of end-stage cases, the majority of subjects affected by ACM who achieve complete ethanol abstinence functionally improve [33,82,135]. The percentage of effective abstinence achievement on these patients submitted to specific programs ranges from 50% to 60% [8,9].

Beriberi heart disease
Palpitations, dizziness, and syncope are common complaints and are frequently caused by arrhythmias (eg, atrial fibrillation, flutter) and premature contractions. In the setting of acute alcohol use or intoxication, this is called holiday heart syndrome, because alcoholic cardiomyopathy is especially dangerous because the incidence is increased following weekends and during holiday seasons. Ask any patient presenting with new heart failure of unclear etiology about their alcohol history, with attention to daily, maximal, and lifetime intake and the duration of that intake.

How soon after treatment will I feel better?
Cardiac catheterization in these patients may reveal normal coronaries or mild coronary artery disease not significant enough to explain the extent of myocardial dysfunction. Accelerated coronary atherosclerosis, coronary vasculitis, coronary spasm, or coronary thrombosis can also be seen in cocaine-related heart disease. The majority of the echocardiographic studies performed on asymptomatic alcoholics found only mild changes in their hearts with no clear impairment of the systolic function. For example, a slight increase in the pre-ejection period/left ventricular ejection time ratio (PEP/LVET) was found by some authors, suggesting a sub-clinical impairment of systolic function[21,33]. Mathews and Kino found a small, but significant increase in left ventricular mass in individuals consuming at least 12 oz of whisky during 6 years and 60 g of ethanol per day, respectively[22,40]. More recently, Lazarevic found a modest increase in end-systolic and diastolic left ventricular volumes and a subsequent thickening of the posterior wall in a cohort of alcoholics consuming at least 80 g during 5 years[23]; however, no differences in systolic function were observed.

3. Ethanol-induced Heart Fibrosis
